Family Advocate Duties:
- Coordinate patient and family support programs
- Maintain regular, effective communication with stakeholders
- Develop and implement strategies to increase patient and family satisfaction
- Promote the identification and implementation of best practices to improve the quality of services delivered
- Develop, implement, and evaluate quality assurance protocols
- Manage patients and families
- Develop and implement strategies to reduce re-admissions

Family Advocate Responsibilities:
- Collaborate with families, community members, and social service agencies to develop, implement, and monitor the transfer of services for at-risk families
- Develop and implement individual and family plans that meet individualized needs of at-risk families and clients
- Maintain appropriate documentation, charting, and reporting
- Assist with the implementation of family-centered practices and programs
- Assist with development of family resource materials
- Assist with coordination of activities related to community services
- Assist with coordination of training activities
- Assist with coordination and distribution of newsletters and other outreach and communication materials
- Assist with data management activities
Requirements And Skills:
- Bachelor’s degree in social work, counseling, psychology, or related field
- Master’s degree in social work, counseling, psychology, or related field
- 5+ years of experience as a social worker/counselor
- Strong knowledge of human services policy, protocol, and practice
