Job Brief:
We’re looking for a Case Manager who can help us with our current caseload.
Case Manager Duties:
- Communicate with patients and their family members to discuss patient needs, goals, and medical history
- Advise patients on available resources, treatment plans, recuperation methods, and techniques
- Schedule follow-up appointments, evaluate the effectiveness of treatment plans, and adjust treatment plans as needed
